Scholarship Recipient Form

Congratulations on being selected as a recipient of Central California Blood Center’s Scholarship! Your scholarship will be held by CCBC and paid directly to the college, university or other learning establishment that you choose to attend. The scholarship payment will be mailed and credited toward tuition costs associated with your post-graduate education. Please indicate your anticipated school start date in this form. Scholarship dollars cannot be given in any form of cash, are not redeemable for cash and cannot be transferred to another individual.
